Breast feeding results in atrophy of the gland, often manifesting as a deflated appearance. In these cases, the most common type, breast implants alone will re-establish pre-pregnancy aesthetics by simply replacing lost volume. More complex cases, like this one, also require removal of stretched skin to provide a simultaneous lift. When skin is removed it only takes a small implant to restore upper breast fullness.
For the surgeons: Since implant size is small in these cases it typically represents less than 40% of total breast volume. Therefore the “feel” advantage of silicone implants typically becomes moot. Saline implants in this setting are reliably stable for 20 years or more and are therefore preferred (125cc saline implants in this case).

Facelifts work well in older patients too. While decreased skin elasticity and facial fat atrophy pose challenges, the usual surgical techniques work quite nicely. First time facelifts in this age group can achieve more dramatic changes than even those seen in younger patients, while still achieving an equally natural result.
For the surgeons: SMAS-ectomy worked great in this patient due to a great bony framework, light soft tissues and mild soft tissue descent. Perioral resurfacing of the central lower face (dermabrasion) a critical adjunct here. Lip lifts (not done here) also common in this age group where the columella-lip distance is often 20 mm or more.

Abdominoplasty procedures constitute a trade: a flat abdomen for a hip-to-hip scar, and one around the navel. How the scars heal is one of the biggest variables. Favorable genetics, fair coloring, and time are key factors promoting excellent scar quality, surgical technique surprisingly less so. A few exceptional outcomes are shown here.
