An excerpt from a recent podcast with my friend and colleague Dr. Gary Linkov. In this segment the topic of fat grafting for breast augmentation instead of using breast implants is discussed. Spoiler alert: it does not work! Despite breast implant limitations they remain the best option.

This is a typical result one week after surgery. Because surgery is performed in a deeper plane (unlike facial surgery for example) there is scant bruising. There is mild swelling that makes the breast appear shiny and taut, but not much larger then they will ultimately be. They are slightly fuller on the top and the sides early on.

We know it’s right when patients say “it looks great but I hope it goes down a bit” rather than “this looks great, I hope it’s not going to go down at all”. Working closely with our patients when simulating surgery with implants placed in a bra beforehand avoids the latter scenario. Additionally, decades of experience allows us to confidently recommend sizes based on goals, patient height, and baseline breast volume.

This patient has a 300 cc implant. She chose silicone and these were placed “under the muscle” through a 1.5 inch incision in the breast crease, off to the side. She is pleased, and so are we!

We were quoted in this well written article about the “internal bra”, a new synthetic mesh material that is shaped like bra cup. It’s intended to prevent stretching of the breast skin by placing it inside at the time of implant placement. It dissolves in about 18 months and the theory is that it promotes collagen formation to make the breast skin permanently stronger.

A word of caution is in order though. It has been promoted based largely on anecdotal experience by a few experts, some with ties to the manufacturer. There have been no clinical trials or long term studies to prove its value. It’s the hot new device though, and many are jumping on the bandwagon regardless. We have seen this before in plastic surgery, where initial enthusiasm over a new product or technique fades as the truth comes out over time. The “internal bra” may prove to be a godsend, but for now the jury is out.

Plastic surgery does not stop the clock, just resets it. At its best the process leaves no fingerprints. Upper and lower eyelids are commonly treated together so the rejuvenation is uniform. This is one of the easiest procedures for patients to navigate: small scale, minimal discomfort and presentable by two weeks or less.
