Out of Town Patients

Our practice is privileged to treat many patients who come from outside our geographic area. The process begins by contacting our office and supplying photographs of the area of interest. Facial photographs should include front and side views as well as a close-up view of the eyes. Breast photographs should include a front view and both sides. Oblique views are not essential but helpful. Abdominal views (front and side) should include the entire abdomen from the bottom of the breasts to the upper thighs. Liposuction views should extend from the navel to the knees and include front and back views. Examples of the different views preferred are found in the Gallery section for each procedure.

Lighting is important. It should be neither too dark nor harsh, and the subject area should be evenly lit. Phone pictures will produced distorted images when taken too close.  This is particularly true for the nose. Facial photographs should be taken with a completely neutral expression. Breast, abdomen, and lower body photographs should be taken with the arms at the sides.

After Dr. Hidalgo has evaluated the photographs, our patient coordinator will arrange a virtual consultation.Following this, surgery can be scheduled and our staff will assist with local accommodations. Lab work, and an EKG, if necessary, can be performed at home prior to travel.  The results should be sent well beforehand though, should any results require further study. Our nurses will coordinate your preoperative care and provide you with detailed procedure specific information beforehand.

Dr. Hidalgo will see you the day before surgery. This live consultation is important to review your goals and examine you prior to surgery. It also provides time to contemplate any adjustments in the surgical plan proposed at this meeting.

If you arrive alone we will recommend a nurse to stay with you at least overnight for most procedures, but up to 48 hours for facelifts and abdominoplasties. Travel home often can often take place 10 days after surgery but two weeks provides a greater safety cushion for more extensive procedures.

International Patients

Our practice treats patients from all over the world, including Europe, South America, India, Africa, the Far East, and Australia. The process is much the same as that described above. Due to differences in time zones it is most convenient to first communicate by e-mail to obtain preliminary information. Although we can make a surgical recommendation based on digital images sent, a supplementary virtual consultation is preferred to best define goals and formulate a customized plan.

Travel from afar has safety implications due to the propensity for blood clot formation in the legs associated with long flight times. Our practice recommends 12 hours on the ground in New York for every hour of flight time. This enhances safety by establishing an appropriate mitigating time interval prior to undergoing anesthesia. Travel home two weeks after surgery is generally recommended.

    This patient is shown three years after her surgery.  Long term results are the best measure of a successful outcome. This case highlights the subtle but critical contribution that a small chin implant makes to the final result.  It evens the facial plane from top to bottom and by itself makes the nose appear less prominent. An extra small size is most commonly used in women and it is a “one and done” procedure. Rhinoplasty is complex and time consuming. Every imperfection is visible “on the table” and the procedure cannot conclude until all is perfect, regardless of the time necessary.

    Lower eyelids typically require more maneuvers than upper eyelids for both to look evenly rejuvenated. Skin quality is more visible on the lower eyelids. So besides removing loose skin it may be necessary to improve its texture with either a laser or chemical peel treatment at the same time. Older patients with weak lower eyelids may require a tightening procedure in the outer corner. This patient is actually a candidate for that but this would change his eye shape considerably, a prospect outside our goals. Instead he had only a temporary tightening to get him through the swelling period.  In contrast, young patients with familial bags typically just need them removed as a single maneuver.  In any event, regardless of what is involved, patients generally look good in two weeks after upper and lower eyelid surgery.

    Rhinoplasty has evolved and is technically more complex today despite its simple conceptual nature. The nose has three structural parts: the upper third are bones, the middle third are cartilages that affect both appearance and breathing, and the lower third are the tip cartilages. Changes are typically made to all three areas. This is shown in schematic representation here but the actual changes made are much more extensive than indicated. It takes a lot of time, effort, and finesse is to create a pleasing and elegant shape, with no margin for error!  Rhinoplasty surgery is truly a labor of love.
