Facial rejuvenation must be an all encompassing plan in order to achieve a harmonious result. In this case a small chin implant improves the length of the neck line from the side while making the plane of face even from top to bottom. The effect on the front view is negligible except for more balanced lighting between the cheekbones and chin. Treating the upper eyelids establishes a clean look to compliment the new neck and jawline without changing eye shape in any way. Lifting the cheek SMAS layer separately allows maximum tightening in a vertical direction to erase the jowls and restore volume while the overlying skin is shifted more obliquely backward. The upper part of the cheek incision should disappear straight up into the scalp and not be positioned below and in front of the sideburn where it is visible. Not every patient needs an aggressive “deep plane” lift. Our patient here, who graciously allowed us to share her images (thank you!), has lower cheek laxity with jowls and deep nasolabial folds that continue below as marionette lines (front view). The upper cheek contours are excellent so not every facial ligament needs to be released in order to effectively move the sagging lower cheeks upward and backward. A “SMASectomy”, a very limited deep plane variant, was all that was needed here. Note how the nasolabial folds are softened and no longer continue downward as marionette lines. Note that the cheek mass is now a smooth entity and the jawline more defined (oblique view).
The upper eyelids are also trimmed to provide an evenly rejuvenated appearance to the face.
