A deep plane facelift, meaning extensive SMAS mobilization with release of facial ligaments, was done here in conjunction with upper and lower eyelid surgery. The intent is to always produce a refreshed look devoid of surgical stigmata. The chin implant provides a subtle enhancement beyond restoration alone. Our patient, kind enough to let us share these images, is thrilled, and so are we!

Breast augmentation is conceptually simple but there are nuances that must be considered in order to get the best results. Chest wall “foundation” issues are not common but sometimes are quite subtle and easy to miss. There are other skeletal issues like scoliosis that can also be impactful by making the breast position look uneven if not taken into account.
For the surgeons: The description of implant characteristics was a bit murky in the video. To clarify, having the same implant diameter was important but at the same time more projection on the low side was needed. A Mentor Moderate 225 implant was selected for the “high” side and a 300 cc Moderate-Plus implant for the “low” side was chosen. Both have the same diameter. Saline was used so that the volumes could be fine tuned for closest symmetry (240 cc right, 350 cc left). Finally, a periareolar incision was chosen because precise siting of an inframammary incision beforehand would have been problematic.

Arm lifts, called brachioplasty, is often viewed more as a general surgery procedure by surgeons rather than a finesse plastic surgery procedure. While it is indeed technically crude compared to a rhinoplasty, for example, there is nevertheless a demand for precision. Setting the final scar position requires precise planning and execution. When the incision is positioned on the very back corner of the arm, like shown in this case, it is not visible from the front or the back, only when the arms are rarely extended above the head. While massive weight loss patients are generally happy and not concerned about the scar, precise placement makes a big difference that is always appreciated . Older, thin women with mostly wrinkled skin pay more attention to scar location and quality . That said, perfection should be pursued for both groups.

It’s hard to take a lot of credit for this beautiful result when there were so many favorable factors: strong bone contours, particularly a wide jaw shape, holds the skin taught like the stretcher frame does for a painting canvas; great “fabric” (good quality skin) adds luster, softens contours, and preserves facial shape; thin soft tissues (cheeks) re-drape easier, reveal underlying bone contour better, and also contributes to result longevity.
Age is another factor but not notable here as this patient is in the normal age range for facelift surgery (mid 50s). Anyone can look like a genius by operating on 45 year olds, a growing trend today. The vast majority of 45 year olds don’t have enough aging changes to justify taking the whole face apart IMO.
